Syml DynaForms Patient⁺ is a secure digital solution to collect quality information from patients via a simple touchscreen within the practice premises. GP practice authenticated users engage with patients by allocating specific chronic condition monitoring, at-risk assessment or survey questionnaires or any other customised information needs. Patients complete their pre-assigned forms digitally prior to their consultation. On submission the information is available instantly on the clinician’s desktop, saving essential clinical time. The clinician can seamlessly upload the information to the respective patient’s record within the existing practice GP system.
